Many couples ask why IVF is not 100% successful in first attempt.

The success rate of IVF depends on egg, sperm and uterus quality. If all the factors are ideal still the chances are not 100% as implantation is a very complex process having thousands of steps which are at the cellular level and cannot be controlled. The potential of young eggs and sperms is better than older persons even though the grading is equal.

As we know that IVF success is not 100% and may need multiple attempts and modifications. Depending on the failure cause if any then we should rectify the problem and proceed for the next cycle and couple may be given options like third party reproduction if needed. Certain modifications can be done in an IVF cycles for optimizing outcome in couples suffering from failed IVF attempts 

  • Embryo Glue: Embryo glue is a special embryo transfer media to improve the implantation and to increase pregnancy rate and clinical pregnancy rate.
  • Intravenous Immunoglobulin (IVIg): IVIg seems to directly affect NK cell level and activity, by reducing their absolute numbers and increasing the expression of inhibitory receptors CD94 which potentially can improve pregnancy outcome.
  • Laser Hatching: Laser hatching of the embryos is performed on the day of embryo transfer to ensure that the shell of the embryo hatches easily. This allows the embryo to implant better. This is mainly suitable for embryos with thick shell, advanced age group and frozen embryos.
  • Dual transfer: If good number of embryos are available, embryo transfer can be done on day 3 and day 5 but it increases the multiple pregnancy rate.
  • Endometrial scratching: It will be done one cycle prior between day 21 to 25 of period to induce local trauma which improves the uterine lining in subsequent cycles.
  • Hysteroscopy: The hysteroscope aids us in picking up uterine abnormalities which are sometimes missed at routine ultrasound e.g., small polyps. It is also useful in washing and cleaning the uterus which sometimes may help in improving the outcomes. Endometrial scratching is also done at the same setting to improve the uterine receptivity.
  • PGTa: PGTa is a way of detecting abnormal embryos thus may help in improving the pregnancy rates. However, each case must be individualized.
